目的 探討丁苯酞氯化鈉注射液治療血管性癡呆的臨床療效。方法 選擇2014年1月—2016年12月襄陽(yáng)市中心醫(yī)院神經(jīng)內(nèi)科收治的血管性癡呆患者100例，所有患者隨機(jī)分為對(duì)照組和治療組，每組各50例。對(duì)照組給予常規(guī)抗血小板聚集、改善腦循環(huán)、降脂穩(wěn)定斑塊、神經(jīng)保護(hù)、調(diào)控血壓血糖等對(duì)癥治療。治療組在對(duì)照組基礎(chǔ)上靜脈滴注丁苯酞氯化鈉注射液，25 mg/次，2次/d，滴注時(shí)間≥50 min，用藥時(shí)間間隔≥6 h。兩組均連續(xù)治療28 d。觀察兩組的臨床療效，比較兩組的簡(jiǎn)易精神狀況檢查量表（MMSE）、蒙特利爾認(rèn)知評(píng)估量表（MoCA）、日常生活能力量表（ADL）和波士頓診斷性失語(yǔ)癥檢查（BDAE）評(píng)分情況。結(jié)果 治療后，對(duì)照組和治療組的總有效率分別為74.0%、92.0%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P< 0.05）。治療28 d后，兩組MMSE、MoCA、ADL和BDAE評(píng)分均顯著升高，同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義（P< 0.05）；且治療組這些觀察指標(biāo)明顯高于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P< 0.05）。結(jié)論 丁苯酞氯化鈉注射液治療血管性癡呆具有較好的臨床療效，可顯著改善患者的認(rèn)知功能，促進(jìn)語(yǔ)言功能和日常生活能力恢復(fù)，提高患者的生存質(zhì)量，安全性較好，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Butylphthalide and Sodium Chloride Injection in treatment of vascular dementia. Methods Patients (100 cases) with vascular dementia in Xiangyang Central Hospital from January 2014 to December 2016 were randomly divided into control and treatment groups, and each group had 50 cases. Patients in the control group were given conventional anti-platelet aggregation, improved cerebral circulation, lipid-lowering, stable plaque, nerve protection, regulation of blood pressure and blood sugar, and symptomatic treatment. Patients in the treatment group were iv administered with Butylphthalide and Sodium Chloride Injection on the basis of the control group, 25 mg/time, twice daily, infusion time ≥ 50 min, medication interval ≥ 2 h. Patients in two groups were treated for 28 d. After treatment, the clinical efficacies were evaluated, and the scores of MMSE, MoCA, ADL, and BDAE in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 74.0% and 92.0%, respectively, and there was difference between two groups (P < 0.05). After treatment for 28 d, the scores of MMSE, MoCA, ADL, and BDAE in two groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Butylphthalide and Sodium Chloride Injection has clinical curative effect in treatment of vascular dementia, can significantly improve cognitive function, promote the recovery of the language function and the daily living ability, and improve quality of life, with good safety, which has a certain clinical application value.
