目的 探討醋酸去氨加壓素注射液聯(lián)合醋酸奧曲肽注射液治療肝硬化合并食管胃底靜脈曲張破裂出血的臨床療效。方法 選取2013年1月—2015年6月在陜西省中醫(yī)醫(yī)院就診的肝硬化合并食管胃底靜脈曲張破裂出血患者96例，將所有患者隨機(jī)分為對(duì)照組和治療組，每組各48例。對(duì)照組患者給予醋酸奧曲肽注射液，首先0.1 mg靜脈推注5 min，隨后以0.6mg溶于5%葡萄糖500 mL中，通過(guò)輸液泵以50 μg/h連續(xù)靜脈滴注，12 h/次。治療組患者在對(duì)照組治療方案的基礎(chǔ)上靜脈注射醋酸去氨加壓素注射液0.3 μg/kg。兩組患者均連續(xù)治療3 d。觀察兩組的臨床療效，記錄患者的止血時(shí)間和輸血量，比較不良反應(yīng)發(fā)生情況。結(jié)果 治療后，對(duì)照組、治療組的總有效率分別為75.00%、93.75%，兩組總有效率比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組患者的止血時(shí)間和輸血量均顯著短于對(duì)照組，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.01）。對(duì)照組、治療組的不良反應(yīng)發(fā)生率分別為37.50%、10.41%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.01）。結(jié)論 醋酸去氨加壓素注射液聯(lián)合醋酸奧曲肽注射液治療肝硬化合并食管胃底靜脈曲張破裂出血具有較好的臨床療效，可縮短止血時(shí)間，降低不良反應(yīng)，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Desmopressin Acetate Injection combined with Octreotide Acetate Injection in treatment of liver cirrhosis with esophageal and gastric variceal bleeding. Methods Patients (96 cases) with liver cirrhosis with esophageal and gastric variceal bleeding in Traditional Chinese Medicine Hospital of Shaanxi Province from January 2013 to June 2015 were randomly divided into control and treatment groups, and each group had 48 cases. Patients in the control group were given Octreotide Acetate Injection, 0.1 mg iv injection in 5 min firstly, then 0.6 mg Octreotide Acetate Injection added into 5% glucose 500 mL, through the infusion pump with 50 μg/h continuous iv infusion, 12 h/time. Patients in the treatment group were iv administered with Desmopressin Acetate Injection on the basis of the control group, 0.3 μg/kg. The patients in two groups were treated for 3 d. After treatment, the efficacy was evaluated, hemostatic time and volume of blood transfusion in two groups were recorded, and incidences of adverse reactions between two groups were compared. Results After treatment, the clinical efficacies of control and treatment groups were 75.00% and 93.75%, respectively, and there were differences between two groups (P<0.05). After treatment, the hemostatic time and volume of blood transfusion in the treatment group were significantly shorter than those in the control group, and there were differences between two groups (P<0.01). The incidence of adverse reactions of control and treatment groups were 37.50% and 10.41%, respectively, and there were differences between two groups (P<0.01). Conclusion Desmopressin Acetate Injection combined with Octreotide Acetate Injection has good clinical curative effect in treatment of liver cirrhosis with esophageal and gastric variceal bleeding, can shorten hemostatic time, reduce the incidence of adverse reactions, which has a certain clinical application value.
