目的 探討復(fù)方丹參滴丸聯(lián)合馬來(lái)酸桂哌齊特治療不穩(wěn)定型心絞痛的臨床療效。方法 選取2014年1月—2014年12月中國(guó)人民解放軍成都軍區(qū)總醫(yī)院收治的不穩(wěn)定型心絞痛患者120例,隨機(jī)分為對(duì)照組和治療組,每組各60例。對(duì)照組患者靜脈滴注馬來(lái)酸桂哌齊特注射液,8 mL/次,稀釋于10%的葡萄糖注射液或生理鹽水500 mL中,1次/d,滴注的速度為100 mL/h。治療組在對(duì)照組基礎(chǔ)上口服復(fù)方丹參滴丸,10丸/次,3次/d。兩組均連續(xù)治療12周。觀察兩組的臨床療效,同時(shí)比較兩組治療前后心功能指標(biāo)包括心臟指數(shù)(CI)、心排出量(CO)、左室射血分?jǐn)?shù)(LVEF)、左室收縮末期容積(LESV)、左室舒張末期容積(LEDV)和血液流變學(xué)指標(biāo)包括全血黏度、血漿黏度、血細(xì)胞比容、血小板聚集率、纖維蛋白原的變化情況。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為73.33%、91.67%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P< 0.05)。治療后,兩組患者的CI、CO、LVEF均顯著升高,LESV、LEDV、全血黏度、血漿黏度、血細(xì)胞比容、血小板聚集率和纖維蛋白原均顯著降低,同組治療前后差異有統(tǒng)計(jì)學(xué)意義(P< 0.05);且治療組這些觀察指標(biāo)的改善程度優(yōu)于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P< 0.05)。結(jié)論 復(fù)方丹參滴丸聯(lián)合馬來(lái)酸桂哌齊特治療不穩(wěn)定型心絞痛療效較好,可改善患者的心功能和血流動(dòng)力學(xué),具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical efficacy of Compound Danshen Dripping Pills combined with cinepazide maleate in treatment of unstable angina. Methods Patients (120 cases) with unstable angina in Chengdu Military General Hospital from January 2014 to December 2014 were randomly divided into control and treatment groups, and each group had 60 cases. The patients in the control group were iv administered with Cinepazide Maleate Injection, 8 mL/time which was diluted in 10% glucose injection or physiological saline (500 mL), once daily with the drip speed of 100 mL/h. The patients in the treatment group were po administered with Compound Danshen Dripping Pills, 10 pills/time, three times daily. The patients in two groups were treated for 12 weeks. After treatment, the efficacy was evaluated, the changes of cardiac function indexes including cardiac index (CI), cardiac output (CO), left ventricular ejection fraction (LVEF), left ventricular end systolic volume (LESV), and left ventricular end-diastolic volume (LEDV) and blood rheology indexes including whole blood viscosity, plasma viscosity, hematocrit, platelet aggregation rate, fibrinogen before and after treatment in the two groups were compared. Results After the treatment, the efficacies in the control and treatment groups were 73.33% and 91.67%, respectively, and there was difference between two groups (P < 0.05). After treatment, CI, CO, and LVEF in two groups were significantly increased, and LESV, LEDV, whole blood viscosity, plasma viscosity, hematocrit, platelet aggregation rate, and fibrinogen were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). The improved degree of those observation indicators in the treatment group was better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Compound Danshen Dripping Pills combined with cinepazide maleate has good clinical efficacy in treatment of unstable angina, and can improve cardiac function and hemodynamics, which has a certain clinical application value.
