目的 對天津市腫瘤醫(yī)院2013年門診抗菌藥物的處方進行分析點評, 為科學(xué)管理和促進臨床合理應(yīng)用提供參考。方法 調(diào)取2013年天津市腫瘤醫(yī)院門診抗菌藥處方共1 579張, 對抗菌藥物的使用進行點評和分析。結(jié)果 門診抗菌藥物的使用人群年齡集中于50歲以上;用藥科室亦較為集中;抗菌藥物處方占總處方數(shù)的0.60%, 品種集中在喹諾酮類和頭孢類, 口服抗菌藥物的使用比例高于注射用藥;點評的不合理處方153張, 占抗菌藥處方總數(shù)的9.69%, 其中適應(yīng)癥不符和用法用量不當(dāng)分別占38.56%、26.14%。結(jié)論 針對本院門診處方中抗菌藥物的使用情況和不合理用藥現(xiàn)象, 提出有針對性地加強處方醫(yī)師的培訓(xùn)和獎罰、優(yōu)化抗菌藥物的品種等干預(yù)措施, 進一步提高門診抗菌藥物的合理用藥水平。

Objective To investigate the conditions of antibiotic prescriptions in outpatient department of Tianjin Medical University Cancer Institute and Hospital in 2013, and in order to provide a basis for scientific management and promoting the clinical rational use of antibiotics. Methods By extracting 1 579 antibiotic prescriptions in outpatient department of Tianjin Medical University Cancer Institute and Hospital in 2013, the usage of antibiotics was reviewed and analyzed. Results Outpatients used antibiotics were focused on the patients who are over 50 years old. And the administration departments were relatively concentrated. Antibiotic prescriptions took 0.603% of all the prescriptions. Quinolones and cephalosporins were mainly used in outpatient antibiotics. Oral approach was used more than the usage of venous approach. The amount of the irrational antibiotic prescriptions was 153, taking 9.69% of total antibiotic prescriptions. In those prescriptions, the cases of inconsistent indications and irrational usage and dosage were 38.56% and 26.14%. Conclusion With the utilization and irrational drug using phenomenon of antibiotic prescriptions in outpatient department, more work should be done to improve the level of rational use of antibiotic drugs such as strengthening the training and reward or punishment of prescriber, and optimizing the varieties of antibiotics.
